A medical note is a document produced by your physician that explains to the insurance company what happened during the medical treatment. It may be a hand-written note by your doctor or an electronic medical record. It should have the following information:
- Date of the visit
- Description of the symptoms
- Beginning of the symptoms
- Proposed treatment/procedure
- Signature by the attending physician
In order for your claim to be processed, you should always get such a document when you seek medical assistance.
