Attending Physician Statement

This is a report by a doctor, hospital or medical facility who has treated, or who is currently treating, a person that is insured or seeking insurance. It can be used by the underwriting department in the insurance company to decide on your application for insurance or to decide if they will accept a medical claim or not.
